The Source Health platform works in partnership with General Practitioners and key service providers to treat people suffering with a comprehensive range of mental health concerns.

Step 1 of 3 - Guidelines

As we want to care for your patients as best as possible, please follow our quick and easy Referral Process when making a referral. As Source Health is a Telehealth Service, we are not able to accept Referrals of patients who are actively suicidal (medium to high risk) or in need of in-patient mental health hospital admission. Only proceed if you are confident that a Telehealth service is appropriate for your patient.

Referral Details*
Medicare requires a VALID Referral for the purpose of issuing Rebates. Please ensure your Referral includes the following, as per Medicare Guidelines:
- Patient’s name and surname
- Patient date of birth
- Patient address
- Diagnosis
- Request for services / treatment
- Referring GP’s Medicare Provider Number
- Referring GP’s signature
- The number of sessions required (e.g. 5)
- Processing of a Mental Health Care Plan
